Lunch Waiver
I, the undersigned Parent/Guardian, acknowledge and understand that students will be given breaks during which time they will have the opportunity to be unsupervised to obtain snacks, lunch or other items outside of the premises of West Metro Driving School. By signing this form, I acknowledge and agree to indemnify and hold harmless Cobb Subs, Inc. d/b/a “West Metro Driving School” from any and all claims or liability which are brought by, or on behalf of My Student, and which are in any way connected to the choices that I have made for My Student (or that My Student has made themselves) to do during breaks throughout the course.
Graphic Images
I, the undersigned Parent/Guardian acknowledge and understand that during the course of instruction at West Metro Driving School My Student may view videos and other media depicting vehicle collisions, accident scenes, car wrecks, and other depictions that show the consequences of failing to use proper and safe driving techniques. I acknowledge and understand that such videos and other media may be graphic in nature showing intense images of violence, blood, and gore. I acknowledge and understand that the viewing of these graphic videos are for the purpose of educating and informing My Student on the dangers and consequences that his/her actions may have on themselves and on others and is an integral component of My Student’s driver’s education course at West Metro Driving School. I hereby give consent for My Student to view the aforesaid videos and other media containing graphic images.
Photography Waiver
I, the undersigned Parent/Guardian, hereby authorize the capture, use, reproduction, and publication of photographs or video taken by West Metro Driving School or its agents that include my image or likeness, or the image or likeness of My Student, without compensation. I and My Student understand this material may be used in various publications and/or websites, for any lawful purpose, including, but not limited to, publicity, advertising, marketing or journalism. I and My Student acknowledge that this material may also be cropped, edited, “photoshopped,” retouched without my further approval. I and My Student acknowledge that this authorization is continuous and we will not be asked to review any final photographs or video before reproduction or publication. I and My Student forever release and hold West Metro Driving School harmless from any and all claims or liability arising out of the use of the aforesaid photographs or videos in any manner or media whatsoever, including, without limitation, any claims for invasion of privacy or publicity.
Privacy Policy and Terms and Conditions for Email and SMS
I, the undersigned Student, acknowledge and understand that West Metro collects and uses personal information to provide and improve its services, which may include sharing data with employees, contractors, and trusted partners. I also acknowledge and understand that West Metro may share my personal information with partners that offer services and promotions which may benefit me. I acknowledge and understand that my contact information, including phone numbers and email addresses, may be used to send service updates, reminders, and promotional communications via phone, email, or text message. I consent to receive SMS messages from West Metro Driving School, its trusted partners, and the technology platforms used to deliver services. SMS communications may include both service updates, promotional content, and other commercial communications. I understand that I can opt out of receiving SMS messages at any time. Defensive Driving (Driver Improvement) Terms of Agreement
I, the undersigned student, agree to complete the above course, consisting of 1 classes for 6 hours each, totaling 6 hours of instruction by the above-name driver improvement clinic. It is understood that this Clinic is certified by the Georgia Department of Driver Services (DDS) in accordance with Georgia Law Title §40-5-80 (DRIVER IMPROVEMENT ACT) and the rules and regulations adopted there under and that each instructor is certified by the DDS. This course is approved by the DDS.
The student's successful completion of the above-named course requires each of the following:
1. Attendance at all classes sober and free from illicit drugs.
2. Attendance on time for all sessions.
3. Reasonable attentiveness and participation in all classes.
4. Attendance at all sessions unless medically excused.
5. All sessions must be completed within 60 days.
6. Successfully passing a written or oral examination with a grade of at least 70.
This driver improvement clinic will not refund any tuition or part of tuition if the Clinic is ready, willing, and able to fulfill its part of this contract. I understand that if I fail to comply with the terms and conditions of this agreement, I am in breach of contract and the school will not be under any obligation to fulfill the terms of this contract, and may, at its option, terminate this agreement immediately.
It is agreed that an owner, instructor, or employee of this Clinic shall not give the impression to a student that upon completion of their instruction this Clinic will guarantee the securing of a driver's license to operate a motor vehicle. However, immediately upon the student's successful completion of the course as described above, the Clinic agrees to provide certification of said completion to the student.
This Clinic has and will maintain for the protection of the contractual rights of the student a performance bond in the principal sum of ten thousand ($10,000.00) dollars for the students to be written by a company authorized to do business in the state of Georgia.
This agreement constitutes the contract between the above-named driver improvement clinic and the above-named student and no verbal statements will be recognized.
